Bleeding in gut: Black, tarry stools or melena is a sign of bleeding in the GI tract.  This can occur anywhere from the mouth to the anus, but in patients with cirrhosis, the stomach and esophagus are primary sources.  See your physician right away for further workup that may include blood work and upper/lower endoscopy.

Sorry to hear: of your husband's trouble...the MOST IMPORTANT "start" dealing with this situation is the complete PERMANENT CESSATION of ALCOHOL in any form! Much of the liver damage may be REVERSIBLE .. Your husband should seek help with DETOX...(If he is already SOBER...vitamins and good nutrition and time may lead to reversing the symptoms/signs . If he continues to drink he will die!

: You need to be examined to confirm what you have observed because stage 4 liver cirrhosis is serious and hallucination due to hepatic encephalopathy may be present.

Every time a patient: With cirrhosis and ascites who develop GI bleeding, the patient should be evaluated by his gastroenterologist asap. If you have any neurological symptoms like lethargy, dizziness, confusion, headache, tremors, rapid heart beating, near fainting, sweating please go to an emergency room asap. Also if the bleeding is profuse.

EGD is next.: People with cirrhosis can get varicose (dilated veins) within the esophagus which can bleed.  If the colonoscopy didn’t show anything then the next step should be EGD for a look at the esophagus.  Blood testing as well to see if you’re anemic and/or coagulopathic.

Don't hesitate to seek urgent evaluation/care especially if lightheaded, dizzy, weak. Sources of particular concern: bleeding stomach ulcers, ischemic colitis, infectious colitis, inflammatory bowel disease. Get right to a doctor, &consider stopping any non-prescribed NSAID drugs now.
