Calculus ?: If the spots are behind your front teeth then it is likely calculus formations. See your Dentist for evaluation and your hygienist for professional cleaning. Ask for strategies to help prevent the calculus (tartar) buildup in the future. Some form it much faster than others. If not calculus, then can be demineralization/decay. Same plan. Good Luck.

Spots for 20y: After 20 years, you want to treat them? You need a diagnosis first.  Go see a dermatologist to see what you have!

AGE SPOTS: These sound like "age spots" or "liver spots". They are usually benign lesions that appear to be "stuck on".  However, some can be pre-cancerous and deserve to be evaluated by a physician. See your primary care provider or a dermatologist.

Tinea: Tinea versicolor usually occurs in teens, it is a fungal infection of the skin, commonly occurs on neck, back and chest.  There are different treatment options including oral medication, but the first line should be 2.5% selenium sulfide shampoo/lotion.  Of course see your PMD for definitive diagnosis and treatment.

Throat problem: There are many problems that can occur in the throat, so get checked.  Spots of all colors can occur.  A brown spot may simply represent dried mucous or prior difficulty with a nose bleed.  Could be irritation from viral or bacterial infection although these are usually red or orange.  You could have some non-infectious irritation from, say, post-nasal drainage (allergies) or even from reflux.

Diagnosis: You noted an enlarged painful lymph node.  Your doctor gave you antibiotics. You now have brown spots in your throat.  Your doctor gave you a diagnosis.  You should ask what that was and write it down.   For a painful enlarged lympn node your doctor is concerned about a bacterial infection.  Maybe from an injury or a bite?  If the lymph node pain goes away that is good.  Follow up is important

Sputum culture: I would suggest testing the sputum for blood , bacterial and fungal pathogens. The sputum may also be part of TB screening. Your PCP can further evaluate and a chest radiograph is also indicated. 
Please seek medical care.

It may not be: Due to sweat.  If i had to say, it may be pigmentation increases that may be due to elevated insulin.  Not sure but if you have a family hx of type 2 diabetes, say, and could stand to lose weight, look up acanthosis nigricans to see if that fits ur description.  Good luck, it's improvable if that is the case, may consider endocrinologist but see pcp first.

Fungal infection: More than likely you have a superficial fungal infection called tinea versicolor.  

Treat with otc antifungal creams such as lotrimin or micatin (miconazole) twice daily for four weeks then retreat one day a month to keep it from recurring.    If this does not work see your dermatologist.

Acanthosis nigricans: Acanthosis nigricans can look like a velvety darkening of the neck with thickened skin folds and increased skin tags. This can be seen with obesity and Insulin resistance, so go to a doctor to have this evaluated.
