Probably neither...: It is not clear what causes broken heart syndrome other than a broken heart (intense emotional stress).   It is more common in women and diabetics and thought to be related to high levels of adrenaline during such reaction.  I am not sure it is correlated to genetic or life style abnormalities.  If you have diabetes please do everything in your power to control it.

Broken heart syndrom: Broken heart or stress cardiomyopathy is caused by stress of break up or death of a loved one.So theoretically loss of a life partner you love and leaves you and you still love can contrbute to stress cardiomyopathy.This condition gets better with treatmnt and is reversible.

Stress: While it is not known for sure, there is speculation about the effects of a sudden surge of stimulants in blood such as adrenaline and nor-adrenalin, leading to dysfunction of part of the heart muscle (specifically, part of the left ventricle actually).  This condition is usually reversible, we do use beta blockers most often to slow down the heart and lessen the presumed effect of the stimulants.

Yes: Also called takotsubo's cardiomyopathy, it is a kind of acute weakness of the hearth muscle in which the  apex of the heart tends to "balloon" thus making the heart resemble an octopus trap ( or tako -tsubo) although associated with significant stressors like death of a loved one, etc.. It can be seen in the absence of these. It causes heart failure but may spontaneously improve.
