In the absence of these signs, infection is unlikely. Pain in the arm can originate in the neck in case there is pressure on the nerves. You should ask your doctor to give you some pain medication and continue to monitor your pain. More tests may be requir.

More info and see MD: Itchy skin may be associated with dry skin--happens moreso in winter or in people who swim a lot in pools.  Persistent chest infection-external or internal--pneumonia or skin infection.  
depression should be addressed with your physician.
swollen glands may be associated with a viral process, infection or an oncologic issue.  Do you have fevers or recent infection and which glands are swollen?

Ear infection: May have an ear infection-viral or bacterial.  Viral infections can last up to 10 days and symptom treatment and time are your best allies. Watch for clear secretions that change colors after symptoms have been present for a week or so and are accompanied by new onset fever or any other symptoms that concern you to decide if you need to see a doctor.

If it is severe, check with your doctor for confirmation.
