Most likely: You mean you have defect in your skull waiting staged procedure, to fill the gap in the skull, if it is the problem, then you must clear with your neurosurgeon,  most likely he will not allow, for fear of injury to brain.

Depends: Usually if there is a burr hole, there is some sort of 'dent' if a plate was not used over the hole.  In skinny patients this can be prominent.  However, hair usually grows there and one can no longer see it but can still feel it.  The scar doesn't go away completely. All the best!

Fracture/loosening: If you have "soft" bone, then the implants may loosen over time.  Furthermore, during the surgery, you are at a higher risk for fracture.

Have it evaluated.: The fusion will limit your mobility of the joint.  The joint replacement will last 10-20 years depending on your weight and level of activity.  One would need to do a thorough history and physical exam and review the x-rays to give you your best options.
