Is it an infection? : Clear jelly like 'pus' is not typical of bacterial infection. Is it something else? Perhaps a ruptured ganglion. I would suggest seeing a podiatrist if possible or any doctor if not.

YES: Is serious. Probably his blood sugar level is too high.
He can have the boils infected very easily and the problem gets worse.
He should immediately be put under antibiotic therapy and have blood test. All under medical supervision.

Paronychia: This sounds like a paronychia which is a common infection of the soft tissue that surrounds fingernails and toenails.  Since it can abscess, it can be very painful.  It is often caused by picking at or trimming the cuticle area.  The best treatments include having a doctor clean the abscess drain the pus pocket.  Antibiotics help, but it sounds like you may need to have the area drained

Exam: It is difficult to know without looking at it, if it is a regular blister from friction or if it is caused by infection. If it Is an infection it would require drainage and antibiotics. Soak the area and keep it clean. Protect it and keep her comfortable. Follow up with her MD or urgent care.

A boil can occur: Anywhere there is hair growth. It is a bacterial infection that develops by the hair follicle. In my practice, i tend to see them on a foot from people using public jacuzzi's. Shaving is another way to get a boil. They are usually pretty painful and doctors usually drain them and if the infection is bad you get placed on an antibiotic.
