Often, : Often, if there is a pattern of holding in of bowel movements, there is an identifiable source of pain or discomfort.  One common cause is the presence of anal fissures (tears in the skin around the anus) which can hurt during bowel movements.  In some cases, there can also be anatomical (structural) issues which cause the bowel movements to have a more difficult time passing.  The best starting point would be an evaluation with his pediatrician to evaluate for the reason he is holding his bowels.  Often, guidance will be for use of medication to loosen the stool to the point that pain is not derived and develop a pattern of comfort and trust in regular production of bowel movements.

ive had old blood/mucas in my stool a few times this month. inconsistently though about 1x a week. i'm not sure if it makes any difference but i have been dieting too. i'm in my 20's and havent long had a child (8 months). do i need to be concerned?

: Any abnormal bleeding, including in stool warrants an investigation. If may be as simple as hemorrhoids, but even bleeding hemorrhoids can cause anemia. It would be prudent to see your primary care provider for an initial assessment and to ascertain if a specialist consultation is needed.

The blood is likely due to either a small hemorrhoid or a fissure that tore and has caused some minor bleeding with the passing of stool. You need to be much more aggressive with treatment of her constipation. Much more fiber in diet, increase water intake and consider med like Miralax (polyethylene glycol).

Couple of causes: In children that age, a rectal fissure is a common cause of bleeding following a bowel movement. Often they have some stool withholding, leading to large stools. This can lead to a fissure when passed. A polyp can cause a large amount of painless bleeding in children. If it has been going on for a while, i would suggest seeing your doctor.

Many causes: There are many diferent cause of chronic abdominal pain in children, most common are constipation, acid reflux then there are less common reasons like ibd, celiac dis. Now bright red blood in stool means it's not coming frm his stomach at least it;s local. Better see your doctor to figure out.Family hx very important.
