See below: Try to leave it intact.  Soak in warm water with epsom salt to dry it out.  Apply antiseptic and bandaid.

Blisters: The most important thing to remember about blisters is this; never pop them.  As long as the blister is intact, it is a sterile environment.  The best treatment for a blister is to apply betadine to it 2-3 times a day until it dries up.  Once it dries up it will separate and fall off.  This will speed up the healing process.

Needs to see: See podiatrist for properly diagnose and treat, could many things, including ulcer, abcess/infection or soft tissue growth. Was any trauma involved?

Nope: If your not a diabetic or immunocompromised , if you have a sterile instrument pierce the roof of the blister and allow the blood to drain. The bandage it up. If you are afraid or unsure how to do this see a foot doctor.

Big differnce: Blister is fluid filled, usually resulting from excessive friction, swelling or other trauma. Gangrene is dead tissue as the result of infection or no circulation. This leads to loss of affected area: amputation.
