BLISTERS ON SCROTUM: Without actually seeing these, they sound like angiokeratomas, harmless lesions that often appear in older men, but can appear at any age.
Also known as fordyce's disease, they do not require treatment.  They can be removed very easily by a dermatologist. This can be confused with fabry's disease, a somewhat serious disorder. Do see a dermatologist if they bother you.

Angiokeratomas: Angiokeratomas of Fordyce appear as deep blue small bumps on the scrotal skin.  They bleed easily as they are dilated blood vessels.  They are harmless and usually do not require treatment.  They can be frozen, burned or lasered off if need be.
If they are not like the picture here, it would be best to see a dermatologist for further evaluation.

there is probably no good way to prevent them as they are genetic, but there are good treatments.  He should see a dermatologist who will help guide him.

You can but: yes you can but to be more effective you need a warm compresser it might be able to control them and get rid of them
the cold will cause constriction of the bloos vessel and make it look smaller as well 
our experience with this king of lesion warm might work better

Need exam: The scrotal lesion could be an ulcer which needs to be looked at to make sure that it is not malignant. There are many other conditions that can cause a bleeding blister like lesion on the scrotum. You need to see either your pcp or a dermatologist for management.

Try OTC: If you don't have redness, pain, swelling, pus or a fever, you don't really need to see a doctor for the toenail fungus. OTC anti fungal topicals can be tried; effectiveness is spotty.There are anecdotal reports of using Vicks VapoRub. Most effective is oral anti-fungal drug treatment, but it requires a prescription and carries some risk. The tiny bit of blood on the penis/scrotum is likely benign
