: At your age it is almost certainly a hemorrhoid.  Best to have a GI doctor check it out and see about removal since it is bleeding and is causing you to worry.

Hemorrhoids: Hemorrhoids are dilated rectal veins in the anal canal that can bleed every time there is a passage of stool. As they worsen, the hemorrhoids can prolapse outside of the anus. See a MD for assessment and Rx.

Should get an exam: A cervical polyp could bleed after a bowel movement, so that's one possibility. Cervical polyps are common and they often recur.  I'd recommend getting an exam and if there's no polyp visible, and no other concerns (infection, inflammation, or abnormal Pap smear) then I'd recommend a pelvic ultrasound to look deeper.

Internal piles: Symptoms suggestive of piles bleeding likely internal as you also had hard stools earlier. Internal hemorrhoids can cause painless bleeding. Recommend increase water and fiber intake. Avoid irritants like caffeine and spicy foods. Regular exercise and adequate sleep. Good bowel hygiene. If persists please see a colorectal doctor.

The presence of any symptoms just listed should prompt an immediate trip to the nearest emergency room without delay.
