Constipation: If you have the urge but cannot move your bowels,insert a rectal dulcolax suppository or a fleets enema. Sometimes manual disimpaction or full tap waster enema is needed. If you have no urge at all, take oral laxative like fleets phosphosoda, milk of magnesia or dulcolax tablets. Inner sphincter: The term 'overactive bladder' is used to describe dis synergy of the bladder muscle -not contracting in coordinated fashion; this leads to loss of regularity and coordinated, controlled urination.  The inner sphincter is controlled by the autonomic nervous system and is involuntary. The outer sphincter is part of the voluntary nervous system, and we can control it (the basis of kegel exercises).

Please Clarify: The sphincter of oddi is a normal anatomic structure at the point where the bile duct enters the intestine (see duodenal papilla on pic above). Dysfunction of the sphincter, though very rare, can simulate gallbladder-type pain & is a possible explanation for ongoing pain despite gb removal. This can be diagnosed by endoscopic testing & treated by cutting the sphincter.

Clarify please: Tamsulosin is frequently used in cases of Hypertrophic prostate.  Is this your cause?  You may benefit from increasing dose but discuss with urologist first

Urethral structure : since you know you have a urethral stricture I assume you have been seen by and evaluated by a urologist in the past.  Any regimen involving self dilation of the urethra should certainly be guided by a urologist.
