This is: likely from polycystic ovarian syndrome/insulin resistance (PCOS).  Increased androgen production. Testing can be done with your doctor. The Mirena is not the cause, it is helping as yu have noted. Endometriosis is made worse by PCOS, and the mirena is helping to counter that. Decreased simple carbs can help and medications from your doctor are possible.

Squeezing pimples: I would not recommend squeezing your blackheads. You can make matters worse. A simple benzoyl peroxide cream or wash can be the first step to improving your acne. Your doctor can also help with prescription medications. Remind your doctor that acne is cruel and unusual punishment and ask for help! (remember cruel and unusual punishment is not ok in the usa!).

Cyst on back: It could be a cyst on your back and the blackhead is the connection to it.  I recommend evaluation by a medical doctor as a cyst needs to be excised surgically.

Doctor can examine: If the doctor diagnoses acne, over-the-counter benzoyl peroxide gel or cream is helpful. If desired, the doctor can add an antibiotic gel or cream such as Cleocin (clindamycin). If more help is needed, retinoids such as Retin-A or Differin gel can be used with or instead of benzoyl peroxide. Oral antibiotics such as tetracycline can be used also.
