Nummular eczema: Prolonged rubbing/ scratching causes dark and leathery skin. Responds to potent prescription cortisone cream. See a dermatologist. Meanwhile try OTC hydrocortisone cream.

: Try an emollient like vaseline in case the lesion is due to dehydration of the skin.
For healthy diet: http://www.choosemyplate.gov/., exercise 30 minutes/day, drink enough water daily so your urine is mostly colorless, have safe sex, no tobacco alcohol weed or street drugs.

Back pain: Degenerative disc decease in the low back is very common at the Lumbar 4/5 level. The pain that  you describe is from impingement of the L5 nerve root. This pain is also described as sciatica. The black patch is unrelated to the radiating leg pain. See a neurosurgeon for treatment.
