Large ones: Usually the 20-21 gauge needles are needed due to the thickness of the testosterone solution.  It can be given in smaller needles but more difficult to inject.

Depends: First you need to have a dentist, nurse, physician that is really good at giving them.  They can actually give them so you don't feel hardly anything at all.  Next, with someone who it good, the most you will feel is a pinch for only about 1-3 seconds.  Then realize that you can stand most anything for only a 3 second duration.

The pharmacy has it: For intramuscular injection the thinnest needle that will not clog is about a 22 guage. 
You can also take B12 liquid under the tongue (need to hold it there for 30 seconds) or take little pills that dissolve under the tongue.

i got an im injection on my right buttock in mid march and started feeling intense pain at the site of injection by march ending till date. the site also seems to have reduced in size. the nurse says the needle had rust on it. could it be tetanus?

No : No not tetanus 
You probably have an injection cyst or an abscess
This occurs occasionally when an injection is given into the Buttock ( gluteus ) and it does not adequately dissolve 
With time and applying heat ( hot water bottle or wheat bag ) might resolve the issue .
If still painful with a significant lump then you will need an ultrasound

i been doing self aspiration of my hydrocel for 2 years now. atlease once a month. what is the best size needle to use? the needle im using is very small with a 2oz tube and i normally drain 6ozs each time but very slowly each time.?

: I wouldn't do this at all. With each attempt there is a chance you will introduce infection into the site which might start slow &amp; eventually move into your abdomen and  be life threatening. Don't expect a licensed professional to be an enabler to such  a practice.

21 or 23g.: I agree that 25g may be unnecessarily small.  21g or 23g would be better.  Pain comes less from the size of the needle and more from the technique of the injection.

There are a few: Techniques that can work fairly quickly. Cognitive behavioral therapy using progressive desensitization (learning to relax ; then imagining the phobic event from least to most anxiety provoking image while relaxed. I usually do this using hypnosis as a relaxation technique, but you can use other relaxation methods. Find a therapist ; you will be able to get relief fairly quickly.
