Cream(s): I usually recommend a nitroglycerin-based ointment.  Also, some pharmacies can make up their own concoction with the collaboration of your doctor. This is called a compounding pharmacy. Perhaps talk with your local compounding pharmacist to see if they have a formulation that is helpful. It is vitally important that you find out why you're having this problem and prevent it/let it heal. Good luck

Umm: Well some anal fissures require surgery. Use the treatment recommended by your physician. If you have not seen a physician then it is best to be evaluated and treated appropriately for treatment.

Don't know: i would call your surgeon. You really don't want to risk an infection with the wrong cream.

Neither: The best treatment for a chronic anal fissure is fiber, a good pain regime, and possible chemical or surgical sphincterotomy to eliminate spincter spasm and allow healing.

small open wound on lower butt crack. not a anal fissure or pilonidal sinus. must have scratched it open. no blood, a small tear in the skin. no infection. what is the best way to promote healing? also, should i apply some form of cream?

: Suggest something like Aquaphor healing ointment 3 times per day. If no improvement by then, you should see your dermatologist. Be certain to keep this wound clean to avoid secondary infection.

Anal fissures: Most important is prevention by eating a high fiber diet that helps produce easy to evacuate stools.
Good hygiene can help with comfort. Clean with baby wipes or by using water in a shower or douche.
OTC meds that can help include hydrocortisone ointment (don't use the cream as it can sting) on the external area and shoved up the anal canal a bit.
If not improving well in a few days, see your doc.

They can help, : But by themselves will not heal a fissure. You need to correct your diarrhea or constipation and maintain a regular bowel patter if there is any chance of healing without surgery.

Pure vaseline: Pure vaseline is best and has little chance of an allergic reaction.

Yes: Yes.  Reduces inflammation.   Keep stools soft.
