Works fast: it works fast ,actually the dose is 10-20 mg before each meal, I will definitely start with 10 mg if this the first time user, to make sure there is no side effect. work much better with IBS  associated with loose BM.
adjusting diet, eat multiple small meals, avoid food that usually caused problems, and that based on each individual separately, exercise, and meditation, and stress management.

See a physician: Reading between the lines it would appear to me that you need more than just a dose increase. You gastroenterologist should re-evaluate  your diagnosis and whether your current treatment is appropriate.

The highest dose: Is 40 mg 4 times a day. But by that time I usually try something else . You don't list your mess or what you have tried. Hyoscyamine  SL would be something to consider

: This would depend on many factors, like the condition of your bowels (for example, how constipated you are at baseline), how well you metabolize the medicine, what you are treating with the medicine, etc. If your constipation continues, or if you are very symptomatic, you should let your doctor know since there are medicines to treat constipation. A GI doctor may be needed as well. Good luck!

: Yes, please take your maintenance medications.  If you are on immunosuppressive medications please check with your specialist.  A few medications should be rescheduled due to the vaccine.
