Diaper: You cant unless you voluntarily  release your urine/bowels.  Your body holds its urine until you allow the muscles to relax so you can void, same for stool unless you are having diarrhea.

Embarrassing: You are to be commended for donating blood. Next time you donate, tell the nurse about your fainting episode so that they may take precautionary measures. It would help to drink plenty of water before donating blood. No harm done by wetting yourself, other than the embarrassment.

Possibly, yes: Receptive anal sex can damage the anal sphincter, which can cause fecal incontinence. The frequency of such problems has never been studied, but it makes sense to avoid trauma:  good lubrication, avoid "fisting", preferably avoid sex toys like dildos. Of all sex practices, anal sex is highest risk for HIV:  know partners' HIV status, use condoms, consider PrEP.

More going on?: Sounds like mom is going through a lot of changes which may be scary for you. If she is incontinent maybe she also has become forgetful (dementia) or has other neurologic or medical problems. Ignoring needles sounds like a sign of a more general problem. I dont want to guess but hydrocephalus is a treatable cause of dementia and incontinence. What does the doctor say?

Assisted : Living facilities may have rules, and incontinence may be a problem their al facility does not deal with, and requires higher level of care. I'd tend to not want to go to a facility that does'nt have the skill level to help and care.

Maybe: How old is the person?  There are many causes of incontinence in adults, but if it is overflow from sleeping too long, them awakening to take them to the toilet might be a good idea.

Nerve damage: The most common cause for permanent incontinence is some type of nerve damage. The nerve damage could be traumatic,  such as a severed spinal cord,  problems with back where nerves are compressed, or disorders that affect the signals from bladder to brain, such as Multiple Sclerosis . Yours may be related to asthma, pertussis, a recent illness, allergy, ACE-inhibitors, smoking (if you smoke, stop now), or other causes.  Rarely cough is related to a type of heart disease. See your doctor for pulmonary function test and chest X-ray to start.
