Yes: Tia's are essentially strokes that don't progress to permanent damage. So any TIA is a risk factor for stroke.

Since the basilar artery provides blood to the back of the brain, those brain structures get temporarily deprived of blood.  Symptoms can be dramatic with total blindness, followed by vertigo, slurred speech, and confusion.  Full recovery is the rule.

: Basilar artery fenestration is a congenital anomaly typically of no significance, but rare strokes have been attributed to its presence.  There does not appear to be any risk secondary to international flights, due to presence of pressurized cabins adjusted usually to no higher altitude than 5,280 feet.  (Do check, however, whether your destination is a COVID-19 "hot-spot".)

is it normal for pulse oxygen level to gradually drop from 99% to 80% over several hours during a basilar migraine attack? or are there other reasons causing the drop? is o2 a better treatment than triptan in this case?

: I'm dubious of that very low Oxygen saturation. It's possibly falsely low because your peripheral circulation in your fingers was vasoconstricted.resulting in poor perfusion of blood to the fingers.  Triptans can cause vasoconstriction. I would expect you to have symptoms if O2 sat was truly 80%.

i have a basilar artery fenestration without aneurysm. i am 36. i am worried an aneurysm will form or a stroke. ive read that aneurysms for 7% of the time. that seems high when speaking about aneurysms. should i be worried? what do i do?

: You  certainly do not have a fenestration in your basilar artery, By definition fenestration is "a perforation in a structure". You may wish to restate your question or better yet, consult a neurologist.

Basilar skull fx: Basilar skull fracture is usually due to blunt trauma to the back of the head. It has unusual features besides from severe headache. Bruises may appear under the eyes ("raccoon eyes"), there may be bleeding behind the ear drum (only seen with an otoscope), and there may be clear watery fluid (CSF) running out the nose. You can also lose your sense of smell and have severe dizziness.
