why does menstrual blood smell so bad?

It shouldn't be..: There is always some/slight odor to the menstrual blood, but foul odor suggests a possibility of infection--vaginitis/vaginosis. A retained tampon is often the course of very foul odor even without menstrual flow. If you have pain as  well, then more severe infection such as endometritis/pelvic inflammatory disease etc..Consult doc. Good luck.

Oxidation: If you cut your finger cooking at first bleeding would be bright red. As the iron in blood oxidizes it becomes dark red then forms a brown scab as it oxidizes further. The same holds true for vaginal bleeding. The blood oxidizes during transit from the cervix to outside. It gets darker, then brownish black during this transit depending on how long it takes. It oxidizes. Hence, too, the odor.

With the : symptoms you are describing, many things could be going on, cystic ovaries, endometriosis, even absent ovulation. Exam and testing with your doctor is very appropriate.

See a gyn: You should see a gynecologist. You may be going into menopause, you may have fibroid tumor. Heavy bleeding can cause anemia.

See Doc ASAP!: Some women have very heavy periods to the point of getting anemic and needing blood transfusions.  Its fortunately quite rare.   Anemia is only one possible cause of your serious symptoms.  You need evaluation by a doctor ASAP!

Odor &amp; bacteria: Usually a vaginal odor of this kind is the result of bacterial growth. Have your GYN MD assess the vagina for a potential  bacterial infection that was "covered up" by the menstrual flow.

i need to go if i should go to the er i was basically off my menstrual period all of a sudden my lower back starts hurting real bad to the point i could barely walk so i went to the bathroom and i had quite a bit of black stuff i am assuming it was blood

If : If you are having severe pain and bleeding, you should either call your doctor for advice or go to the er.  Even though it could be something minor, it could be a surgical emergency such as a tubal pregnancy or torsion of the ovary.  Better to be safe than sorry.

Spotting : Spotting can be a sign of pregnancy, so you might want to take a home pregnancy test.

You can either take a home pregnancy test as long as it has been 5- 6 weeks since your last normal period, or you can go see your doctor and they can test you. A home pregnancy test is 97% accurate if taken a week or more after the missed period and a doctor's test can tell around when you should be getting your next period.

There are other reasons besides pregnancy that can cause a woman to miss a period such as: stress, diet, excessive exercise or a hormone imbalance such as with a thyroid disorder or polycystic ovarian syndrome. If you continue to have an irregular cycle, you should give your doctor or your ob/gyne a call. Good luck!

Depends: If you have heavy flow with clots some thing is irritating  the lining ( endometrium ), like fibroids, displaced iud etc, see your gyn doctor.
