Pain/drainage: I think the most consistent symptom of an ear infection is pain.  Depending which part of the ear is infected it can also affect your balance.

Drainage of fluid may also be present if it is an outer ear infection.

If your ear hurts or your balance has changed  seek medical attention.

: If there is an oral antibiotic which is sensitive to that unusual bacteria I would prefer a combination of both topical and systemic antibiotics for this otitis media because of your concern. Good luck!

Ear drum hole: How do you know how big the hole is? Did an ENT see you?  Most estimates of the ear drum size are inaccurate unless done by an ent.  The good news is most small ear drum perforations heal beautifully and you just have to give it time.  If it does not heal in a few months  most holes can be easily fixed by placing a paper graft over the tympanic membrane after freshening up the edges.  Good luck!

: CRP is just a marker for inflammation factors in the blood. It does not distinguish what the cause of that inflation would be. ( there are several)

The treatment for scarlet fever is the same as strep throat-antibiotics such as penicillin.
