how can i effectively treat my back spasms? it's been 12 hours now and i'm unable to get any relief. i've been unable to sleep because it's impossible

Spasm: Lay flat on your back with your knees bent - this is a position that allows the muscles to let go.

Put a pillow under your knees (sometimes 2) and see if it will just let go.

i have extremely bad back spasms at night only that stop me from sleeping and then ache all day. what could this be?

Degenerative disc : Degenerative disc disease, disc herniation, facet disruption, all of the above.

can i take a baclofen, if i had taken a tramodol 2 hours ago?  i am having 10 out of 10 on the pain scale back spasms.  i just n back has been spasming for about 6 hours.  some positions help but not for long.  they just seem to come in waves.

Tramadol : Tramadol is usually dosed every 4-6 hours.  Baclofen is usually every 8 hours.  They can both cause respiratory depression so should definitely only be taken as prescribed.  You could also try alternating heat and cool packs to your pack, and taking advil (ibuprofen) (it's not strong but sometimes the combination can help) if you don't have reasons you can't take nsaids (blood disorder, ulcer, etc).

Hypnic jerks: Probably hypnic jerks and periodic limb movements of sleep. These are worsened by some meds, caffeine, stress, alcohol, sleep deprivation.

Hand spasms: Shake your hands to increase circulation; rest hands on pillows; take a cup of soup broth - then take another cup; drink orange juice or orange pineapple juice for potassium and magnesium;

Back pain: This could be all muscle, but you do have risk factors for occult fracture because of your rheumatoid.  TYLENOL (acetaminophen) OR Motrin for starters.  You should have a low threshold to get checked if not improving.

why does a sedative sleeping pill relieve my spasticity but nothing else will?

Depends on what: Ur "sedative sleeping pill" is. A benzodiazepine or muscle relaxant can both be sedating and work well for spasticity. How do you know that you have muscle spasticity?  Why do you have spasticity? Need more information about your history and condition before more salient advice can be given.

does anyone know what causes (i.e. physiology) the extreme fatigue in ms? (no sleep disturbance, have adequate intake and exerc., spasticity treated)

Brief response: Fatigue is very common in MS and typically best correlated with "burden of disease" illustrated on MRI studies.
