i have had on and off back ache (feels like a stomach ache in my upper back).  sometimes it resembles hunger pangs.  this evening i have diarrhea and i am wondering if this pain can be related to something going on in my stomach?

: Could be ... but on this forum there is no way to tell.  Could be trivial (gastritis), could be very serious (pancreatitis) - or anything in between.  If severe and present for more than a day or 2, suggest you go to an Urgent Care for “hands on” evaluation.

See your doctor: This is one of those problems where a visit to your doctor is necessary to figure out what's going on.  Only after a thorough evaluation, including examination and possibly labs and other tests, can your doctor correctly diagnose you and treat you effectively.

Suffering: I have to assume that over the past year you have been carefully examined by your doctor and also specialists.

If that has not been done you should quickly see your doctor

If nothing abnormal has been found despite careful attention I think it would be appropriate for you to have a referral to a Psychologist.

Get a general check: If you have access to healthcare, it will help to see a provider to get a general check up. It appears that your problems may involve several organ systems. This kind of situation is better addressed with detailed history and physical exam. Seeing a PCP will be a good place to start.

Gastric Ulcer: These can produce back pain and various other sequelae such as sepsis and lung injury.  I would recommend evaluation by your doctor.

Symptoms: That is a number of symptoms effecting various organ systems. At your age an anxiety problem would be suspicious, however, perhaps a thyroid or even metabolic problem like diabetes possible.

No: This is concerning for a peptic (stomach) or duodenal (small intestine) ulceration or cholecystitis (inflammation of the gall bladder).  Eating decreases the acidity of the contents of the stomach and intestine and will decrease pain.  It also makes the gall bladder contract so eating usually increases gall bladder pain.  See your primary or a gastroenterologist.
