can i take azithromycin with food and/or antacid?

Azithromycin: You can take Azithromycin with food; however, do not take antacids that contain aluminum or magnesium within 2 hours before or after you take azithromycin. This includes Rolaids, Maalox, Mylanta, Milk of Magnesia, (magnesium hydroxide) Pepcid Complete, and others. These antacids can make azithromycin less effective when taken at the same time.

why can't an antacid be taken when also taking azithromycin?

Binds the medication: Many medications should not be taken with antacids.  Either it will inhibit absorpton by binding or it will change the acid or base of the stomach and make the medication ineffective.  Usually it binds the med and keep it from being absorbed.  Always ask your pharmacist or physcian if concerned.  Very imortant with medications like 
thyroid medicaiton.

i took azithromycin 1gm for chlamydia but after 10 minutes i took digene (antacid) , may i have to repeat the course for chlamydia

Likely not: You should get a test of cure but you probably digested them.

i'm taking azithromycin, i know in supposed to avoid antacids. can i take it with nuun electrolyte tablets which have magnesium sulfate?  i am having diarrhea thus the nuun.

Ok: Diarrhea is a common reaction to Azithromycin but it sounds like you need to be on this medication for treatment of your infection.  If you are having so much diarrhea you feel dehydrated, speak with your doctor.  It is ok to combine Magnesium Sulfate with azithromycin.

i took azithromycin-500mg, 1gm, 1gm for chlamydia, but last two days i took alcohol and also digene tablet(antacid), should i have to repeat the course?

No: Neither of these should affect the zithromax you took.  Be sure that your partner gets checked too and don't have sex with him until you know he doesn't have chlamydia too.  Remember that only condoms will protect you from std's, and even those aren't 100% all the time.

Common Side Effect: Nausea and belly pain are a common side effects of azithromycin at the dose used to treat chlamydia. Taking an antacid that far after the azithromycin dose is unlikely to change the antibiotic effect, so don't worry about that. Please be sure that you were also tested for gonorrhea since co-infection with chlamydia is a common problem. I hope that you are feeling better.

i take ranitidine or omeprazole for reflux.  need to begin azithromycin (z-pack). how long to wait between meds since zith is not good with antacids?

Doesn't matter: Neither ranitidine or Omeprazole are classified as antacids.  Antacids are things like tums, (calcium carbonate) maalox, rolaids, mylanta, etc.  As such, there is no need to wait between taking either ranitidine or Omeprazole and azithromycin.

Test of cure: You took the correct dose of medication. It should be effective. For chlamydia infections i recommend a restest after 3 weeks to make sure that it has been completely cured.

Heartburn: Over the counter medications such as Tums, Rolaids and Maalox are good for the occasional relief of heartburn. Zantac, Prilosec, Prevacid, Nexium, (esomeprazole) Omeprazole, Lansoprazole are more effective for daily use to prevent recurrent heartburn.
