Unclear. : There is a syndrome called polyglandular autoimmune syndrome type 3 that involves multiple autoimmune diseases (hypothyroidism, dm, psoriasis, lupus.....). 
You may still get one or none depending on your family tree.  There is a genetic link with diabetes type 2

Possibly :  It’s possible that an infectious illness can act as an adjuvant stimulate antibody production. There is no investigator proof that this specifically happens.

I : I assume it was your TSH that was 4.2. Your dose of thyroxine is low. You would need to simply stop that medication and have repeat labs checked in 2  weeks. A test of anti thyroid peroxidase antibodies would also be helpful as would an exam to see if your thyroid is enlarged. Check with your doctor to arrange this and watch for hypothyroid symptoms. If you have hypothyroidism there is nothing proven that helps you "rebuild" it. Check out the site below.

NEUROPATHIC PAIN: Diabetic neuropathy comes to mind. Certain chemotherapeutic medications can also be responsible. Do check with a neurologist. Hope you are better.
