How large: This can be  many different  skin rashes. We need to know  the size, is it flat or thickened, is it painful, does  it itch etc. Sometimes a black and blue injury can change to yellow as it heals. It is best  diagnosed  by a dermatologist. Ask your physician about seeing a dermatolgoist.

See pain doc: Localized pain without x Ray findings can be a problem for the patient and practitioners  when cause is elusive. I suspect the real issue is from your cervical spine with pain referral. Get an x Ray with full flexion and extension views of c spine to look for ligamentous laxity. Get trial of local lidocaine injections (or buy 5% topical at pharmacy) to T2 area. Use neck bolster during sleep.

Possible infection: this sounds like an infection it could be from bug bites or some Mild Eczema that has got infected a picture would go a long way

Dermatologist: These will have to be inspected by your physician or dermatologist. An allergic reaction appears unlikely if the spots aren't raised and there is no itching and the spots don't come and go such as hives do. Insect bites also tend to itch and eventually clear.

: Examination by a physician is advisable. I suspect an insect bite with yellowish skin changes probably caused by breakdown products of some bleeding that occurred at the time of the puncture. I'm concerned about the arm swelling which can be an allergic reaction. Presence of surrounding and spreading redness would indicate secondary bacterial infection requiring oral antibiotics.
