No,should not: Elective (  means not as in ruptured one ) aortic surgery has excellent result , with very low mortality. Recently more and more cases are being done by endovascular stents with out opening  abdomen  ( similar to coronary stents)  observation for small aneurysms , surgery only if size is increasing, may be causing anxiety.

Chest pain: Better to get checked with a family history of heart problems.  Ask your local hospital or physicians to help you out in setting up a payment plans.

There is a less invasive "endovascular" sheath in developmental phase that may be.

Depends: Yes if willing to take risk of rupture, then  no one wants to take risk of rupture .   4 to 5 cm   aa will have .0.5 % to 5 % chance of rupture     5 to 6 cm has  3% to  6%   ask your vascular surgeon  he will advise what is best.

5.5 -6.0 cm: Other factors are important:
valve leakage( aortic regurgitation)
increasing size or pain
hypertension( high blood pressure)
other cardiac or vascular, or lung disease
other significant risk factors for a major operation
in general, ct follow up about every 6 months until > 5.5.

Yes: Smoking can cause the atherosclerosis or accelerate it and its also known to cause aortic aneurysm.
