Injury?: Ankle issues in patients your age most commonly develop from a traumatic or overuse injury. Was there an injury or are you exercising/in training? Varies: Varies somewhat on your specifics.  Ideally, if it because of a tear in the extensor retinaculum, then a repair or reconstruction of that structure would be the most physiologic thing to do.  Don't hesitate to discuss with your surgeon.
