Use only Wt. and Ht.: BMI is a screening number, and uses only Weight and Height. Don't add in other stuff to get the BMI number. CDC says:  the accuracy of BMI as an indicator of body fatness... varies depending on the degree of body fatness. In other words, among relatively heavy children, BMI is a good indicator of excess body fatness;... among relatively thin children... BMI is more an indicator of fat-free mass...

if my son is age 12 but has a bone age of one year behind (taken about 5 years ago) would i use age 11 in one of the height calculators?

Not necessarily: A bone age of 1 year behind from 5 years ago could very well have changed by now--could be that the bone age has caught up and now he is a bone age of 12, not 11. Or maybe his bone age is even ahead and he is 13, not 12. You should use his chronological age, 12. And if you are still worried about his height, go see an endocrinologist and have another bone age and workup done.

Dietitian/wt watcher: If you can't resist food, there almost no chance you're going to be able to lose weight consistently.  See doc to rule out other problems. A good doctor, good dietitian, good program such as weight watchers may help.  Also counseling can be beneficial. There are weight loss drugs, but only work while you're taking them. Avoid health food store stuff as it doesn't work. Surgery? Good luck.

Tachycardia: Newly evident rapid heart rates need evaluation to exclude organic pathology which can be myriad eg thyrotoxicosis, anemia, infections, pulmonary disorder, cardiac disorder et cetera.  In the absence of organic underlying pathology it frequently is a reflection of cardiopulmonary deconditioning. See an appropriate D: You fail to describe your menstrual experience. PCOS usually involves irregular or absent menstrual bleeding as well as facial hair, loss of scalp hair, acne, obesity, acanthosis  nigricans, etc. An endocrinologist, especially a GYN endocrinologist, is a good way to go. You should diet + exercise to slim down. You may be a candidate for metformin and/or spironolactone. Do not smoke.

Big for age not obes: Your son is growing as fast as my son. If you fit him on a growth chart he will be at 95th percentile for weight and about 90th percentile for height. Note that your child's bmi is 18.7 which is normal his weight matches his height!

When shopping for fruits, buy small ones no bigger than a tennis ball, because fruits have sugar. Drinks without calories include water and green tea. A calorie is a calorie, but surely avoid sugar & simple starch calories.
