I would evaluate: An infant of that age is at risk of sudden deterioration into an infection caused by germs transferred to them in delivery. An axillary temp of 38 would be a core temp of 38.5  which is ominous at that age. Take him to be evaluated now.

received moderna booster 4 days ago. 3 days ago had arm soreness, tenderness underarm and low grade 100 degree temp. arm at injection still mildly sore, underarm still tender and no temp. how long can i except underarm to stay sore? is that normal?

: Individual variation of symptoms in post vaccinations incident to Pfizer, Moderna and J&amp;J booster shots will resolve usually within 3-5 days.  There is some evidence which supports the severity of symptoms increasing with the time elapsed subsequent to the "fully vaccinated" state (second shot with Pfizer and Moderna).

Watch for now.: I would not do anything t this time. Watch for further symptoms. If the baby is doing ok with eating, sleeping and disposition wait to see how things develop the next 24-48hrs. It the temp is over 101f may consider a fever reducer. If over next 1-2 days she still has a temperature over 100.5 f or higher have the doctor look over things and consider a urinalysis.

Axillary temperature: "A normal axillary temperature is between 96.6° (35.9° C) and 98° F (36.7° C). The normal axillary temperature is usually a degree lower than the oral (by mouth) temperature. whats more accurate for taking temp? oral or under armpit? i took it in my armpit and it read 96.5. five minutes later took it orally and it was 98.1. got nervous when it was 96.5. so dont take it under armpit?

: We consider the most accurate reading as one that comes from your core, with an internal probe. Rectal monitoring is standard in some settings. When you do surface monitoring (skin/armpit) it is less accurate and expected to be a degree or two lower. Oral temperatures are standard for most clinical situations. I wouldn't worry about it.

What : What you should do depends on the age of your baby.  First of all, the most accurate temperature in a baby is a rectal temperature, so you may want to confirm the fever with a rectal temp.  Any fever (temperature over 100.4f) in a child younger than 2 months old is a medical emergency because it can be the only sign of serious illness.  Babies that young should be taken to the emergency room for a "sepsis work-up" looking for infection in the blood, urine and possibly spinal fluid.  They may also be admitted to the hospital on IV antibiotics for 48 hours until doctors can be sure there are no bacteria in the blood, urine or spinal fluid causing the fever.  These infections in such a young child can be life threatening which is why we make such a big deal out of it.

If your child is between 2 and 3 months old it is not necessarily an emergency, but they should be seen in the next few hours for evaluation.  You will want to call your child's doctor right away to see if they can see you in the clinic or if they think you should go to the emergency room.

If your child is older than 3 months, then take a deep breath - a fever is not an emergency in this age as long as the child is acting ok (see signs to worry about below).  A fever is the body's natural reaction to an infection and the fever itself is not dangerous.  It's more important to treat the child than the fever.  If your baby seems uncomfortable, you can treat him with Acetaminophen (tylenol) or give a lukewarm water bath.  If he seems fine, you don't have to do anything different - just keep an eye on him.  You may want to call your child's doctor for further instructions or to have him seen during clinic hours.

Reasons to go to the emergency room or call your baby's doctor sooner are:
-refusing to eat
-irritable or lethargic
-if the fever doesn't respond to treatment after 24 hours or is higher than 104f rectally
-diarrhea, vomiting or dehydration
-rash with fever
-stiif neck
-difficulty breathing

use your instincts and when in doubt, just call your child's doctor.

URI- AIR TRAVEL... !: Most probably viral. But at this temperature, better if travel can be delayed. Upper respiratory infection with inflammation/swelling of throat, nose can cause severe earache while plane taking off and landing, due eustacian tube dysfunction. If possible, have a check up with the child's pediatrician.
