Lasers, peels: Post inflammatory hyper pigmentation can occur in skin after injury or acne. The best prevention is to avoid sun exposure (sunscreen) to avoid stimulation of the pigment cells (melanocytes). Once present, bleaching agents such as kojic acid or hydro quinone along with retinoids can help. Typically chemical peels or laser tx is needed for severe cases.

Post inflammatory: Post inflammatory hyperpigmentation is a common and normal response during the healing process.  When traumatized skin heals, redness, followed by dark discoloration often occurs.  With time, the site will usually resume normal coloration to match the surrounding skin.  Avoid picking and overly drying facial products &amp; consider a noncomedogenic concealer.

Takes time: Eventually, if you are not having new lesions, the old red spots will eventually fade to skin color, as long as you avoid sunburn or other injury.  Unfortunately, it may take several years.

Hydroquinone: I'm glad you're getting scientific treatment for your acne. If your current regimen is not sufficient, a systemic rx may be in order. The dark post-inflammatory spots will face in a few weeks but if you're in a hurry, a hydroquinone skin bleach is worth a try.

Salicylic acid: Look for this on your wash. It may help. If it does not clear you, get with your personal physicain who can add something that will.

Let them fade: I do see that you are being treated for seborrhea with ketoconazole, and also for acne Clindamycin monotherapy. It's a serious mistake to treat acne just with an antibiotic without topical benzoyl peroxide or some similar medication. Better management of your active acne will make the question of red and dark spots after acne moot. Good luck.

Scientific Rx: No topical agent is strong enough to control acne on the trunk. Keep using topical benzoyl peroxide and get with your personal physician as well for prescription-strength rx. You may be a candidate for a low-progesterone oral contraceptive pill which may clear you.

Can do all: Acne can leave dark spots. If there is no true scarring then the skin will return to normal. You need to be on acne treatment that will prevent flares. See your dermatologist.

Not sure: I am not sure what "dark spots" you refer to. Some women may have darker spots especially on the areolae (areas around the nipples) that normally tend to be darker than the rest of the breasts. Those dark spots are just extra pigment collections and there is nothing to do about that. If you have dark spots that are bothersome, changing in shape or color, they ought to be checked by physician.

Retin A (tretinoin): Retin a (tretinoin) is a  prescription. The generic, tretoin, is the same but costs less. Tretinoin gel should be obtained in the largest size.  Apply a very small amount every night. At first it may be irritating & drying, so you may skip a night sometimes. It is less irritating if you wait 20 minutes after washing. Expect improvement after 3 months, then continue for at least a year. Frustrating, but it tak.
