Muscle irritability: I believe the pathophysiology behind carpopedal spasm induced by trousseau sign is that the ischemia produced by cutting off blood flow (for 3 min) to the arm brings out the irritability of the nerves in the arm due to hypocalcemia, thereby causing muscle contractions that result in carpopedal spasm.  I do not believe that localized acidosis or a further fall in calcium levels plays a role.

Twitching, facial: Also tingling and numbness and seizure-like episodes in severe disease. These symptoms may suggest hypocalcemia but are not diagnostic. Diagnosis is only done by boos test. Don't treat yourself based on symptoms only without doc supervision since unnecessary calcium intake has its drawbacks too.

A rare condition called tumor lysis syndrome can also show low calcium and high uric acid levels.

In the extreme: Hi. You should be being treated with calcitriol and calcium for your hypoparathyroidism. If you don't take your calcium &amp; calcitriol, your calcium can get very low, and if very low, dizziness can ensue. Be safe, treat your hypocalcemia. I recommend you run your calcium at or just below the lower limit of the normal range, so you don't have too high urinary calcium and risk kidney stones. Good luck

Very unlikely: Hypercalcemia can cause hypercalciuria (excess calcium excretion into urine) which results in higher urine output and may lead to weight loss.
