No: I don't know of any pain patches that contain tylenol or acetaminophen.  They may contain fentanyl, a narcotic, or a NSAID (non-steroidal anti-inflammatory).  To be sure, if you are using over-the-counter patches, read the label carefully.  If you are being prescribed patches, ask your doctor.

Back strain: You likely strained muscles/ligaments in your lower back.  The best treatment is ice or heat to the area, Ibuprofen for pain if you can take this, and avoidance of lifting/bending for a few days.  If pain worsens, or you develop numbness or weakness in legs, or problems urinating or moving your bowels, see your doctor.  Good luck!

Hard question: But if you naive to these drugs it would be a really bad choice to take them.  This is a "generic" answer only.  These are all sedating and habituating and have additive effects.  This combination is usually only seen for palliative treatment for terminal and painful illness and improper use would only hasten that.  I don't know your situation and don't presume to question.

Continue to use: It will take time for you to find a new steady state with your pain. Getting off, or at least reducing, your need for narcotics will be beneficial in the long run. Talk to your provider about specific issues.

Strongest opiate: Fentanyl is the strongest followed by dilaudid, Percocet and narco. However, over time every opiate produces tolerance and withdrawals if stopped abruptly.  They are usually never a long term solution for chronic non cancer pain. Fentanyl hsbc a black box warning to be used only for cancer pain in a patch form. It is used IV for intra operative pain during surgery.

You should be asking: The doctor who is prescribing all of this.  If you are talking one alcoholic beverage (standard drink 12 oz beer, 5 oz ordinary wine, 1 1/2 oz distilled spirits), it would not likely be an issue, although any amount of alcohol may increase the risks of tylenol (acetaminophen) if you are taking larger doses on a regular basis (2 grams per day or more- approx 6 norcos). If larger doses of alcohol, then not ok.

No: Do not take acetaminophen with alcohol.
For good health - Have a diet rich in fresh vegetables, fruits, whole grains, low fat milk and milk products, nuts, beans, legumes, lentils and small amounts of lean meats. Avoid saturated fats. Exercise at least 150 minutes/week and increase the intensity of exercise gradually. Do not use tobacco, alcohol, weed or street drugs in any form. 
Practice safe sex.

Yes: For a healthy individual, it is recommended to not use more than 3 grams in a 24 hour period.
