Thrombosed: Thrombosed hemorrhoids are quite painful until the blood clot is removed. If bleeding is brisk and persistent I feel a colon-rectal surgeon or proctologist is best qualified to treat you.

Not likely a tumor: A brain tumor is pretty far down the list of possibilities. I think you have pinpointed the most likely cause yourself: muscle tension. Neck, shoulder, temple, jaw muscle tightness and pain create anxiety which causes more tightness which causes more pain which ...
   I don't discourage you from seeing your primary care physician but I think you can worry less about a brain tumor. Feel better!

May be misdiagnosed: TLE doesn't contribute or lead to schizophrenia but it might be misdiagnosed as it. An EEG will show it and not schizophrenia. And, if well treated, visual hallucinations will be gone, so to be well Dx'd really matters.  Definitely see a neurologist and/or a neuropsychologist specializing in TLE. It could make a huge difference. If it's you and it's new - it's likely too. 44's a bit old for new Sz

Migraine: Could be an ocular migraine. Is there a family history of migraine or personal hx of other migraine variants? Check your blood pressure to be sure this isn't the cause. The reduced vision is very concerning.  It may just be migraine but you could have increased intraocular pressure (glaucoma), or it could be an indication of something intracranial even more concerning.  You need evaluation.
