Strain: Severe strain with tearing of muscle fibers may take a little longer to heal. Rib fractures can also occur with hard coughing. At this time I would try intermittent heat to the area which may help the healing process. Hold onto the injured area if you must cough or sneeze and avoid other strain as well.

Certain situations: Reasons to drain a hematoma might include enlarging size that suggests ongoing bleeding, infection, compression of adjacent structures, or external bleeding.  If none of these are present, the hematoma will get reabsorbed over time on its own.  Speak with your surgeon if you are concerned about it.

Drainage if possible: Of course I am curious about the origin and exact location of the hematoma. Assuming that it is externally caused and accessible I would recommend incision and drainage, with or without packing. These lesions, as you know, can be painful and even become infected.. Draining it is usually a minor procedure done under local anesthesia. Have her doctor clear her and get it done.

Muscular Pain: The abdominal wall consists of multiple muscle layers.  Like anywhere on the body, a pulled muscle can cause significant, lingering pain. Since we use our core muscles for most activities, it can be difficult to rest these muscles. Pain from the abdominal wall can be difficult to distinguish from "internal" abdominal organ pain; muscular pain tends to be unrelated to food & worsened by activity.

Usually nothing: The opening on the abdominal wall is small and just large enough for the  colostomy to fit.  The edges of the colostomy are then tacked down to the surrounding skin to seal the area.  Rarely does anything go wrong.  Occassionally there might be a paracolostomy hernia that then needs repair or a  prolapse or dropping out of the colostomy itself that needs to be fixed.

The xray can provide information on the status of bowel distension, presence of stones, etc, and the abdominal ultrasound can provide info on kidney, liver, gallbladder, pelvic organs, etc.
