Everyone's different: ...But, generally speaking, 12 month olds will have 2-3 words that they will say. 15 month olds will have about 6-10 words that they will say, 18 month olds 10-15 words, and 2 year olds have 20-50 words, at least. By 2, children will also start putting words together in phrases. If your toddler does not meet these criteria, talk to your pediatrician about evaluation.

Encourage: A child aged 12- 17 months has a 4 -6 words vocabulary . Repeat words , say aloud and encourage him . At this age they imitate words .

Numbers vs clarity: I expect a kid to form some recognizable words by or shortly after their first birthday, but clarity takes time. By 3 they are chatter boxes but only 75% of what they say is clear to strangers. By 4 most will be clear to strangers with a few exceptions.

Speech: Use of unrelated words which are clearly articulated  might occur as part of a thought disorder. If the words used sound like the word that was intended that might be due to malapropism.

I would not be too concerned about it.  Just one part of your brain works faster than another part. The picture is of the Reverend William Archibald Spooner (1844–1930), Warden of New College, Oxford, who was notoriously prone to this mistake.  Although some claim his students made them up.
