my 4yr old son has bloodshot eyes and said his head hurts really bad. so much so it woke him up from sleep. what would you do if it were your child?

Really bad headache: If it were my 4 year old, i would have him see his pediatrician asap.

See Peds Neurologist: I would be concerned about any child awakening/or not from sleep with a headache. You need to see a peds neurologist asap. They may also be behaviorial issues causing this problem-bullying, sexual molestation, school pressures etc that could be at the root of this complaint.

Child question: It may be sleep apnea with your child. The grinding of teth by sleep apnea causes cluster headaches and the bedwetting can come from sleep disturbences. 

Have a dentist eval your child and check tonsils. If swollen tonsils and dark circles under the eyes its time to see an ENT for tonsil and adnoid removal.

Concerning symptoms: These symptoms are very concerning and point to a process in the brain, possibly infection, mass, lesion.  Suggested immediately evaluation by PCP/neurologist, probably needs imaging and labwork.

This is not always something that a partner will hear you doing and usually most people aren't aware they are doing it. Bruxism can be very destructive to your teeth, jaw muscles and TMJ (jaw joint). A proper diagnosis and effective treatment can be achieved from a highly trained dentist.

Maybe: Causes of a headache include sleep apnea and clenching your teeth at night.  I would suggest seeing a dentist or physician that can deal with these issues or other causes of headaches.

Aura: What you describe is an aura. An aura is a scent, a feeling, or an activity of the body that warns you you are about to have a migraine headache. This allows you to take medication to abort the migraine before it can cause you a lot of pain, nausea and discomfort. You should see your doctor to see what medication to take to stop your headache  when your aura occurs. This is not something to fear .

since then ive had a sleep and have woken up still feeling the same.
do these two symptoms bring concern and that i should get a test?

COVID 19 concerns: Suspicion for COVID 19 is higher if you have at least 2 of these symptoms:  temp of at least 99.6 F (37.5 C), Cough, Shortness of breath, Chills, Shivering, Muscle pain, Headache, Sore throat or new loss of taste or smell.

ER: Any change in headache severity or quality after a significant head injury, and particularly if the head injury CAUSED you to go to sleep, is reason to seek urgent medical evaluation.  Again, if you have a headache, and not just pain where you hit your head, that is worsening, please go to the ER.
