Who knows?: Labor initiation will depend upon your previous number of deliveries, and when Mother Nature decides to start the process.  An older OB MD when I trained in OB GYN was most accurate in his old adage, that "If the apple is ripe, it will fall off the tree". We only "pluck apples"(induce labor) when you are post dates or there is a known medical problem with you or the baby.

: I assume you have a heart murmur. I also assume the 65-70 percent refers to the ejection fraction which is normal meaning your heart muscle is strong. You have mild mitral valve regurgitation causing the left atrium to be mildly dilated&amp;also causes mild elevation of systolic blood pressure of the pulmonary artery. You have very mild mitral valve disease which is best followed by a cardiologist.

had my 40 wk checkup yesterday. doc says i'm 100% effaced but not dilated at all. when can i expect labor?

Any day but...: It could be today but could be another week or 2.First babies seem more likely to come a bit after the due date than before. Your baby will trigger the process of labor when it's ready. Do your best to avoid formally inducing labor until at least 41 weeks but having sex, breast stimulation and acupuncture may help get things going. At 41 weeks your doctor or midwife could try membrane stripping.

A long ways to go...: Dilated to 2cm:  The bottom of the uterus (The cervix) is open about 3/4 of an inch.  Fully dilated is 10cm.  Effacement: The cervix is 4-5cm long.  50% effaced means that the cervix has thinned to 2 - 2.5cm.  Station:  When your Ob checks your cervix, he/she can feel a landmark called the Ischial Spine.  When the fetal head is at this point it is at zero station.  -3 Station means 3cm above.

Can not guess: It is not possible to guess when someone will labor exactly. If you are 2.5 cm dilated and 50% effaced all we can say is "soon." We can not say if soon means today, tomorrow or weeks from now. I would keep a bag packed and ready to go though. Best of luck.

I hope not: Hopefully you are writing this from being on bedrest and in consultation with a high risk OB and your own ob. Many things can be done to slow down or stop preterm labor. Every day you can give your baby inside is a huge benefit on the outside and long term. Please avail yourself and your child the best treatments for stopping preterm labor, for maturing baby's lungs, etc, for best outcome.

Concerning: This is worrisome for your risk of pre-term delivery. Your doctor will probably be monitoring you more closely because of this and checking for any contractions. You may even be given some steroids to help with maturing the baby's lungs. This is more worrisome if this is your first pregnancy or if you have had previous preterm babies, miscarriages, abortions, or procedures on your cervix like LEEP
