Uncertain: The yolk sac should be visible on transvaginal sonogram with an 18mm gestational sac diameter, so this may possibly be abnormal.  Check with your doctor, follow up ultrasound might be helpful.

Sac size: Sac size is much less reliable than crown rump length and your fetal heart rate is fine.  Don't worry about the sac size.  Very common to see one either larger or smaller than the fetus.

From a normal pregnancy; so both tests indicated

Irregular sac: An irregular sac with a lower than expected level of hCG suggests that there is a problem with the pregnancy.  As with most cases either it will resolve itself or end of as a miscarriage.  I suggest that you have a serum progesterone drawn as well because low progesterone in a otherwise normal pregnancy can result is miscarriage.

5 wks plus/pregnant: At five plus weeks, it is not always possible to see a fetal heart on ultrasound.  Another ultrasound in "2 wks." (if that is what "usg" means) is appropriate, but if you are having bleeding, it may be a miscarriage. Get a quantitative HCG (serum) from your gyn provider. This can give definite indication as to whether your pregnancy is still viable.

About 1 cm: Usually the sac will be larger. Usually you will start to see a yolk sac then. But this is the dilemma. If the pregnancy implanted 1-2 days later than you think it can alter what you see. So a repeat us 1 week later is much more accurate.

Gestational sac size: What we should see at 7.5 weeks is a developing fetus with cardiac activity and a crown-rump length consistent with the menstrual cycle or fertility treatments.  This is more important than the actual sac size.  If the heartbeat was seen then there is a viable pregnancy.

Patience is Key: It is best to calculate your pregnancy age by measuring the crown rump length which can be done in 2 more weeks after the embryo and heart beat are seen. Measuring Gestational sac is not that accurate for dating a pregnancy. So, please see your doctor in 2 weeks. Take Care.
