Could be BPH: You describe common symptoms of BPH or bothersome urinary symptoms caused by enlarged prostate. Symptoms include weak stream, difficulty starting stream, hesitency, urinary frequency/urgency, nocturia. I would see your medical doctor. He will likely perform an examination of your prostate and may consider starting medication to treat your bothersome urinary symptoms.

Perfectly fine: Taking birth control pills does not have a significant impact on your ability to get pregnant in the future and you become fertile within days after taking your last pill.  On the other hand, it is important to keep in mind that your ability to get pregnant decreases with each year you age.   The decline is slight in your 20's but getting pregnant gets much harder in your late 30's and early 40's.

No: Plan B One-Step is up to 89 % effective when taken within 72 hours (3 days) after unprotected sex. It can reduce chance of pregnancy by up to 95% if used within 24 hours. It continues to reduce the risk of pregnancy up to 120 hours (5 days) after unprotected sex, but is less effective as time passes.

Not likely: Sometimes opks are difficult to interpret, but usually to catch the lh surge you will see a negative test one day, and a positive test the next.  It will remain positive for a few days after ovulation as well.  If you're having mid-cycle spotting, and your OPK turned negative the day after positive you probably had a false positive.  I often have my patients come for day 21 blood draw.

i have been on nexium (esomeprazole) for five years and it just isn't working anymore is there anything i can use over-the-counter that might work?

Nexium (esomeprazole) times 5 years: I would think that it is time to get the underlying cause of this treated. Are you eating foods that do not cause gerd? Are you of an ideal weight or trying to get there? Do you smoke? 

That is where i would focus. Getting to not needing antacids any longer.
