Delayed speech: A 4 year old with autism will have had delayed speech and language. They will also play alone more and not want to interact with others as much as a non-autistic 4 year old will. There are always grey areas so if you think your 4 year old is not developing normally or talking as the should, see your pediatrician as soon as possible. It happens: Toe walking happens in normal kids and usually fades by 2 but some do at at 4 but these kids usually have tight heal cords.It is not a specific feature of autism.Not understanding what she's told and not speaking are worrisome, but more detailed workup would be needeed to apply a reasonalble label.She could simply be deaf.

Flapping arms: Is one of those characteristic complex repetitive behaviors seen in autism.  But children without autism "flap" too.  It is not diagnostic Are there any other concerning behaviors?  If you have concerns you should talk to your pediatrician and have her evaluated.

Not necessarily bad!: Echolalia is also a part of normal language development.   This phase begins around 18 months of age when a child has mastered imitating words and is just beginning to imitate phrases.  Echolalia peaks around 30 months of age, and declines by the time a toddler turns 3. Delayed echolalia is the repetition of words or phrases that are echoed after the fact.  Autism spectrum must be considered!
