Exam: Can be many reasons: strain, kidney stone, arthritis, slipped disc, etc. Take your sister to her obstetrician.

Not normal but seen: Constant back pain limiting basic activities of daily living (e.g. Walking or sitting) isn't normal; not even at 40 week of pregnancy. The weight gain, the "basketball" & surge in hormone preparing pelvic bones for delivery may contribute to the problem. Constant severe activity limiting pain should be discussed with ob. C-section is considered as the condition may interfere w/ vaginal delivery.

Get examined: If you're not pregnant, it's not labor pain.  I'm worried about you though.  You may have an infection, or even another (very early) pregnancy and if it's painful, your life may be at risk if the egg implanted in a fallopian tube.  Please go see your doctor immediately--do not wait.

Many things: Activity like walking can help. Having sex can stimulate the hormones that induce labor, as can nipple stimulation. Overall, maintaining good sleep, and per patterns supports your body's cycles and will make labor easier. Some people feel some over the counter herbal teas that include red clover can help. Although studies are lacking, it should not be harmful.

What did your OB say: No big deal if you are 1 cm, but big deal if you can't stop the labor, please discuss with your OB I am sure your OB wants to try to get u to 37 weeks which is term if u go into labor then, no reason to stop it.

Depends: A normal pregnancy can last from 37-42 weeks, &amp; if the baby &amp; mom are healthy, most Drs do not try to bring on the labor artificially until at least 40 weeks.  Definitely call your wife’s OB provider if any concerns though

Depends: It is reported by some anecdotal reports that arnica montana may be helpful for pain much like an antiinflammatory drug would help. I am not sure if this would be effective enough though, but probably safe.

Breech: Rare to have the baby change from vertex to breech at 39 weeks. Not a lot of room for baby to move at this time except you have a lot of amniotic fluid around the baby which will expand or create a lot of room for baby to flip from head down to buttocks down. It is expected that labor pain will start anytime. If not after 41-42weeks most places in the usa will induce. Good luck.
