Twice : Twice a day is terrific, but the one in the evening before he goes to bed is the most important, since any food and sugars left on his teeth will also feed the bacteria that are there, all night long.

Hard to say: Were they damaged in the accident? Do you go to the dentist twice a year to have your teeth examined and treated? If not, that may be your answer.. Neglect only allows underlying problems to get worse. See a dentist to determine if the last three can be saved to help support dentures.

Yes: The back of lower front teeth is a common area to build up tartar. This tartar is old plaque that becomes hard or calcified from the minerals in saliva. It feels rough to your tongue. This will cause gum inflammation that make them bleed and sore.  Will lead to more serious disease and infections. Gum disease is common in adults but can be controlled. See your dentist.

Until old-age: Adult teeth continue to grow, but very very slowly.  Normal chewing/grinding help to keep the teeth at appropriate length. An unapposed tooth can grow long to cause problem. Excessive grinding, more often during sleep, will wear out the teeth earlier...But your teeth probably don't stop growing until old-age. Healthy teeth=healthier body/heart. See dentist regularly.
