Depends: Were you subject to female "circumcision" as a child? If so, then your clittoris was removed and a major source of sexual stimulation has been lost. If not, then you need to discuss the situation with your partner. Many women do not obtain adequate stimulation from standard "missionary position" intercourse and require additional direct stimulation of the clittoris to acheive orgasm. Good luck.

This is usually...: Psychological in nature. If you have any or many negative feelings about what you are engaging in, this would be one of several factors. Certainly, neuropathy can cause this, but it would first affect hands and feet. Cauda equina syndrome, can cause this, but you would have bowel issues as well. At 44, is this of recent onset? You did not say.
