Probably not: If the sperm are deposited in the vagina - they will naturally migrate to the egg. Penis length is not likely to be a factor either way.

Depends: If he is able to initiate intercourse, then size would not be the problem. 3 inches should be successful. You may need to try different positions, to allow the greatest penetration.
