Specialist: I believe that your best course is to consult a proctologist or colorectal surgeon for diagnosis, treatment and follow up care. You may be dealing with a recurrent anal fissure but I would be concerned about the presence of a perianal abcess which requires treatment by a specialist. In the meantime apply warm compresses or take sitz bath.
