Varies: Irregular bleeding is an almost universal side effect the 1st few weeks after Mirena (levonorgestrel) insertion. After that periods tend to lighten with 25% having no periods by 12 months. Irregular bleeding can be annoying but it does not mean that the method is not working. This side effect is very typical of Progesterone only birth control methods.

ER or doctor ASAP: Its not uncommon to have irregular periods on Mirena; however, you shouldn't have abdominal pain when you bleed. Its more worrisome that the pain is on one side. There are some serious possible causes for left-sided abdominal pain and bleeding such as infection and ectopic pregnancy, so you need to have this checked out ASAP. Best wishes!
