Every : Every woman's cycle is different, so it's difficult to say exactly when she ovulated (time when a woman can get pregnant). However there is a good chance that she did ovulate on or near the 12th based on the date syou gave; any semen (including precum) has the potential to cause pregnancy, especially because it stays inside a woman waiting to fertilize an egg about 2-3 days. The only way to know is to take an early prediction pregnancy test which can be done about a week before her next period is due. I hope everything works out.

Never say never: with sexual intercourse. Don't know how long her actual cycle is. When a condom breaks it is no longer protective. Pre-ejaculate can sometimes contain viable sperm.
