Dyspareunia: Painful intercourse can arise from ligaments of pelvic joint, largest joint in axial spine, the sacroiliac joint. Ligaments are stretched in missionary position as femur leverages & torques the pelvis. Pain generated by sacroiliac joints can radiate to mid-pelvic region, leading to incorrect perception that vagina is problem. Problem particularly in Hypermobility Syndrome. See comments:

My estimation is that about 15% of women are born w. genetic variation of benign Joint Hypermobility Syndrome, JHS. These women are "double jointed" & flexible, w. joints that commonly pop when stressed. Those w. JHS are prone to joint subluxations/ligament injuries, especially sacroiliac & shoulder joints.

Pain in mid-pelvis can be referred pain from pain generator in musculo-skeletal tissues of pelvic girdle. Women told they have "vaginal prolapse, bladder pain, dyspareunia, painful ovarian cysts, PCOS or endometriosis" should seek Osteopath's second opinion & evaluation for possible pelvic girdle joint/ligament disorder.

At Quora.com on my Pain Syndrome board: a bibliography presents more than 100 medical studies of sacroiliac joint. Most physicians unaware this joint has normal range of motion & that SIJ ligament injury & laxity can cause this large joint to generate pain & have abnormal range of motion; absent inflammation & unable to be imaged because is subtle movement disorder; as medical literature reports.
