Infection in scrotum: The epididymis is behind the testicle and provides a place for sperm to mature. When it gets infected or inflamed its called epididymitis. When that happens the entire testicle and epididymis swells, is hot to touch, and really hurts. This is an emergency and needs to be distinguished from testicular torsion (twisting of testicle cutting its blood flow) within 6 hours and then requires antibiotics.

Scrotal pain: Epididymitis is inflammation or infection of the epididymis, the tube located behind the testicle that connects the testicle with the vas deferens. Symptoms include pain and swelling in the scrotum, sometimes with fever and/or lower abdominal pain. It is often caused by sexually transmitted infections, especially in younger men, and always requires antibiotic therapy.
