How to keep an erection while switching positions?

See your doctor: This problem can only be solved by seeing your doctor and being evaluated face-to-face. After a thorough examination, your doctor should be able to tell you what's wrong and what to do about it.

Try not switching: Is it you, partner or both who want to switch positions? Certainly psychologically possible if you are feeling great in original position and don't really wish to switch. Could be that you basically don't like a different position or find it stimulating. Could be due to prolonged erection without ejaculation & are switching to try and achieve orgasm. First try & discuss with partner.
